Treatment of Bone Defects in Lateral Tibial Plateau Fractures Augmented with Allogenic Bone Graft

Abstract
Purpose: To evaluate the results of treatment of bone defects in lateral tibial plateau fractures augmented with allogenic cancellous chip bone graft.

Materials and Methods: Between May 2003 and May 2006, 22 patients with bone defects in lateral tibial plateau fractures augmented with allogenic cancellous chip bone graft were included in this study with more than 1 year follow-up. Out of 22 patients, 16 cases were men and 6 cases were women with average age of 44 years(22 to 66). Clinical evaluation was analysed using preoperative and last follow-up Lysholm Knee Scoring Scale. And at the time we observed the loss of radiodensity line surrounding the bone defects area, we regarded it as the time to radiological union.

Results: In the clinical evaluation, the mean preoperative Lysholm score was 52.4(45~69) and at the time of last follow-up, the mean score was improved to 84.5(67-92) (p<0.05). In the radiological results, the mean time to radiological union was 4.7 months (3.5~6.1), and we obtained radiological union in all the 22 cases. 7 cases had radiological traumatic arthritis at last followup. Out of these 7 cases, 5 cases showed more than 5 mm secondary depression of the tibial plateau. No infections were detected in this series.

Conclusion: In the aspect of the incoporation, allogenic cancellous chip bone graft for a depressed tibial plateau fracture were considered effective. However, secondary depression of the tibial plateau fracture and secondary osteoarthritis were revealed when there were severe comminuted fracture and knee instability due to associated ligament injury.
